What is sufficient proof for overage dependants who are full-time

Q. What is considered sufficient proof for overage dependants who are
full-time students and wish to continue benefit coverage?
A. We require (1) a copy of the dependents current student card, (2) as well as
any statement which identifies school enrollment for the current year. Examples: Financial
statements, transcripts, letter from school administrator. Please ensure the current year of
enrollment is visible or this will be deemed ineligible.
Q. How often should I submit proof of school enrollment for my overage
dependant child(ren) who wishes to continue receiving benefit coverage?
A. We require that you submit proof of school enrollment on an annual basis; again
indicating the current year dependant is enrolled in school. The actual year (i.e 2006 – 2007) must
be visible in order for the document to be considered acceptable. Please remember that upon
students 25th birthday their benefits will definitely cease.
Q. How often am I allowed to withdrawal/transfer funds from my RSP
account(s)?
A. Up to a maximum of 4 withdrawals and/or transfers per calendar year.
Q. Whom do I call regarding specific questions, concerns about my (medical,
dental, vision) claim assessment?
A. Our insurance carrier provides an on-line member service called GROUPNET.
Members can access specific claims information by first registering and then logging into their
member account. Click

access the GROUPNET web portal. Once you login to your account, you will be able to check the
status of claims, see your claims history and more.

Q. How do I withdraw money out of my RSP account?
A. Please go to link- Commonly Used Forms, print out the form, or you can get a
copy faxed or mailed to your address. Complete all the sections of the form, sign and date (spouse
and member must both sign if spousal RSP) and return to The Power Sector Office.
Q. How do I transfer money out of my RSP account?
A. Please go to:

copy faxed or mailed to your address. IN ADDITION, you must complete a
T2033 form. This is supplied from your new investor. We do not provide you with
this form. Complete all the sections of the form, sign and date ( spouse and member must both sign
if spousal RSP) and return to The Power Sector Office.
Q. How do I change my contact information (ie. Address, phone number)?
A. Login to your PSBT Personal Account– change contact information and follow the
prompts.

Q. How do I change my current dependant(s) on my benefit plan?
A. Simply login to your PSBT Personal Account and make any changes
on-line

You can complete the change of dependant form to add/remove person(s) to or from your plan.

Q. How do I change my beneficiary on my RSP account(s)?
A. You must complete the Change of Beneficiary form. This form can be requested
from the Power Sector Office or go to the link
Forms. It will be mailed to your home
address and you must return it to our office.
Q. I have lost my Assure Drug Card. What do I do?

Q. When do any changes I have made to my benefit and/or RSP plan

A. Any changes made to your Benefit/RSP plan, become effective on the 1st of
the following month.
